Habilitative Systems, Inc. (HSI) is a 501c3 mission-driven, multi-faceted human service agency established to alleviate human suffering by developing and providing resources to promote dignity, self-sufficiency, and empowerment for humanity. The organization uses a comprehensive multi-tiered approach that engages prevention, intervention, treatment, research, and care management to build healthy communities. Founded in 1978 by a small faith-based group of concerned community leaders, HSI has a strong history of helping vulnerable, low-income people who face a wide variety of adverse health and social outcomes which include, but are not limited to: homelessness, substance abuse, mental illness, and developmental disabilities. Our core services are segmented into four categories: Residential Services, Disabilities Management, Children and Family Services, and Behavioral Health. Headquartered in Chicago’s westside Austin community, HSI grew from its humble beginnings as a $400,000 nonprofit into an $8 million continuum of care organization that annually serves over 7,000 households. Moreover, HSI expanded its service area to include 15 locations in other predominately impoverished African-American and Hispanic Chicago community areas which include: West and East Garfield Park, North and South Lawndale, Austin, Humboldt Park and Englewood. With an established presence in these communities, the organization employs a staff of 100, 50% of whom reside in the service areas, are culturally and linguistically competent, and are community-focused on transformation and impact. As a result, the agency maintains superior service and program efficiency and effectiveness because of stabilized employer/client relationships. For over 40 years, HSI’s greatest efforts have proven to enhance and to equip the community residents with tools to achieve healthier, robust lives. The organization has been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF, Intl.) since 1991 for its programs and services. This distinguished recognition demonstrates continued commitment, skilled competence, and quality of care to each person we serve. As a Medicaid certified and licensed organization, HSI meets healthcare quality standards, which further details our willingness to undergo rigorous compliance evaluations. As part of our strategic plan for the next five years, HSI seeks to expand services in all Illinois Department of Human Services divisions, Illinois Department of Corrections, Illinois Department on Aging, Illinois Department of Public Health and Illinois Department of Healthcare and Family Services. Analytical data also supports expansion of services to veterans and their families. Additionally, HSI is pursuing Opportunity Zone investment for the development of a veteran’s facility to be located in the Austin community.Relevant People
